  • report a missing pension payment.

 

(NOTE:  Do not call before the 1st day of the month.  If you have not received a check, First State Trust Company will reissue a check after 10 business days have lapsed since the date of mailing.  For example, if checks were mailed on January 25, 2013, then the date your replacement check will be reissued is February 8, 2013). Please contact IPL HR Retirement Services to initiate a check replacement process.

 

  • change your federal, state and/or local income tax withholding amount(s).
  • establish and/or change your direct deposit banking information. 

 

(NOTE: If possible, do not cancel your current direct deposit instructions until your new instructions have been completed by IPL’s Pension Trustee, First State Trust Company.  Also, if a change of direct deposit instruction affects where you are having spousal insurance premiums, if any, automatically deducted by the VEBA Trust, then you must also contact Dunn and Associates to report your banking change.)

  • change your address.
  •  report a name change.
  • report the death of a retiree or the retiree’s pension beneficiary (i.e., spouse, surviving spouse, or Contingent Annuitant).
  • request duplicate income tax reporting Form 1099-R.
  • receive a “Pension Award Letter” for home mortgage or loan refinancing purposes.

  • report any membership eligibility changes for you or your covered dependents, if any.

  • report a retiree, spouse, beneficiary or qualified dependent death (See also “Taxable Death Benefit” section below).

  • receive information about monthly premiums for your eligible spouse, qualified dependents and/or beneficiaries, including changing bank information for the “Auto Debit” premium payment program.

  • report an address change.

2.  VEBA TRUST – TAXABLE DEATH BENEFIT* PROGRAM: *(Applicable for retirements effective before January 1, 2010 only.)

  • request a form to update your beneficiary records.

  • report a retiree death.
